Complaint procedure – information for patients
We understand that sometimes a formal approach is needed to investigate problems. If you wish to submit a complaint about the service you have received from SpaDental, please fill in our Complaint Form (linked below) for further investigation according to our Complaint Procedure.
Before submitting the Complaint Form, we kindly ask you to consider whether the matter can be resolved verbally with the practice manager or is suitable for communication via our Feedback Form.
To submit a formal complaint
If you have not done so already, please familiarise yourself with our Complaint Procedure information before you fill in the formal complaint form.
When we receive your form we begin to investigate your complaint according to the procedure. You may be assured we will look into the matter promptly and thoroughly.
