The consultation form below allows you to book an online video call or voice call with one of our team members. We aim to answer your questions to help you understand fully what Membership is so you can make an informed choice.

How the consultation works:
- Choose a time that suits you. Please note a booking cannot be made within 24hrs. Please use our website contact form for more immediate concerns.
- Fill in your details. We need this information so we can check your clinician’s recommendation. Please remember to fill in your phone number as we will use this to call you if the online link fails.
- Be sure to write your email address. You will receive an email with a link to click on to join the meeting at the scheduled time.
- You can also cancel or reschedule your appointment from the booking confirmation email.
- There is room to write a short comment. Please indicate any particular concern you’d like to discuss so we can be prepared.

The Consultation Procedure
- When you book you will receive an email with the link to join the meeting. If you do not join the meeting, we will use your telephone number to try to contact you.
- If you realise you cannot attend the meeting, please reschedule or cancel your appointment from the link in the booking email.
- Prior to your consultation, consider the questions you have about membership. You may want to note them down.
- Please note:
- this is a remote meeting – you do not need to visit your practice
- it is not a clinical meeting – your booking is with a member of the reception team
A few reminders about SpaDental Membership
- Patients already registered at the practice can become members straight away.
- Patients new to SpaDental must attend a new patient appointment prior to becoming a member.
- When you first register, the plan provider DPAS adds an initial registration charge of £10.00 to the first payment.
- SpaDental Membership also features worldwide dental emergency assistance.
Preventive care helps patients keep their teeth and gums healthy and protects against more expensive and more challenging treatments.
